Health and Developmental History: Specific Visits

Questions for the Health History

Health History ImageA health and developmental history should be obtained from the child's parent or an adult familiar with the child's history. The history can provide information about health problems for which there are no standard tests.

Using open-ended and developmentally-appropriate questions helps providers to obtain more information and invite discussion with the family.

The pull-down menus below suggest age-specific questions on physical, developmental, and behavioral health, as well as topics including family, nutrition, and healthy and safe habits. The 5 - 20 year visits include questions for the child or teen in addition to those intended for the parent.

These questions are intended to be used selectively during health interviews.

From the broad lists of questions offered, providers should tailor each interview by selecting questions most appropriate to the needs and circumstances of the family, child, or teen.

 

0 - 11 Months

Questions in these visits might relate to topics such as the baby’s temperament, response to parents and others, comforting techniques, or forms of play.

 

1 - 4 Years

Questions in these visits might relate to topics such as the child’s sleep habits, means of communication, tantrums, or self-care abilities.

 

5 - 10 Years

Questions in these visits might relate to topics such as the child’s social skills, friends, interests and talents, or ability to express feelings.

 

11 - 20 Years

Questions in these visits might relate to topics such as the teen’s decision-making skills, ability to deal with peer pressure, social activities, and future goals.
